Chronic non-communicable diseases are the biggest global health problem, being the main cause of the high number of premature deaths, the loss of quality of life of a large part of the world's population. The objective was to identify people registered on the e-Gestor platform, with a diagnosis of arterial hypertension and diabetes mellitus in the city of Rio Branco Acre, as well as to describe the cardiovascular and diabetes risk stratification protocol. This is a descriptive quantitative study, using the database of the e-Gestor platform on the coverage of primary care for hypertensive and diabetic people in the city of Rio Branco, Acre. As a result, 253,096 active people were confirmed in the individual registration of the Unified Health System, in Primary Care in the municipality of Rio Branco, Acre; classified as, 247,484 active individuals and 5,612 who left the register (5,009 due to change of territory and 603 due to deaths). Among them, we highlight 6,184 (4.1%) cases of diabetes mellitus and 21,191 (14.2%) cases of hypertension. It was evident that there is no protocol, in practice, for stratifying cardiovascular risk and diabetes mellitus in the routine of Health Units. In addition, the protocols made available in the Primary Care Notebooks of the Ministry of Health are used by health professionals, but there was a structuring of the care line and protocols for stratifying cardiovascular and diabetes risks, which after the evaluation will be improved and implemented in the health units in the city of Rio Branco, Acre.
